Dear Student Athlete,

On behalf of the athletic training department I would like to welcome you to Eastern Oregon University. At EOU we strive to provide you with the best medical care available. As you prepare to join your respective team, there are a few things the athletic training staff will need: 

1. Physical Exam

A physical exam needs to be preformed and filled out before you can participate. Print the physical examination form. Only this form will be accepted. Please attain a physical examination using the following guidelines.

The examination can be performed by a MD, DO, PA, or NP
Can be performed by a family doctor or a doctor that is covered under your insurance if criterion indicated above is met
Only the EOU form will be accepted - no exceptions
Completely and clearly fill-out the form 

2. Medical Forms

EOU medical forms can be found on this webpage under the links: Information Questionnaire, Medical History, and Consent Form.

Complete, sign, and date each form
Return to Athletic Trainers via mail or when you arrive on campus
The forms have to be the originals – no exceptions 

3. Copy of Insurance

Please review the EOU Insurance Policy

Provide EOU with a copy of both sides of your insurance card
Make sure that your insurance is current and provides coverage for athletic injuries, MRI, X-rays, physical therapy, or other extended medical tests
Primary insurance is required to participate in EOU athletics 

4. Release Documentation

To help to avoid delays if there are questions about previous injuries and your ability to play please provide the following:

A Doctor’s Release or a copy of the original release for any injuries that needed surgery
OR any injury that required a release from a doctor to return to full participation
This is required for any injuries you may have sustained two years prior to entering EOU Athletics
